When arriving at the Immigration office in Chaguaramas you will be required to fill out carbon copies of one (1) Arrival Report form, three (3) Arrival Report, four (4) Crew List, One (1) Declaration of Health, one (1) Stowaways List and two (2) Arrival Card.
